Address Change/Permit Lost or Destroyed Form

Within 30 days after a permittee changes the address specified on his/her permit or three business days after his/her permit is lost, stolen or destroyed, the permitte must notify the issuing Sheriff's Office of the change of address or permit loss, theft or destruction.  There is no charge for an address change; the fee for a lost, stolen or destroyed permit is $15.00 to obtain a duplicate permit.
