The following forms are provided for AED Site Coordinators and Project Participants:

-Readiness Checklist Generic v.20 [pdf] This form should be used to maintain a record of regular AED weekly equipment checks.

-AED Use Report [pdf] This form should be completed when the AED is used.

-Estimate of Ongoing Expenses [pdf] This form provides Site Coordinators with information on AED maintenance costs.
